Kansas HealthWave

A representative for Kansas HealthWave will be available to assist parents with questions or signing up.

When:  February 13th
Time:  4:30-8:30pm
Location:  Oregon Trail Middle School

Parents MUST bring with them:

  1. 2 forms of ID for student
  2. Birth certificate
  3. Immunizations
  4. Drivers license
  5. ID for parent/guardian applying
  6. Proof of guardianship -  if not parent
  7. Unearned income statement
  8. Child support
  9. Unemployment
  10. Social security
  11. Pay stubs         
  12. Full month of most recent pay stubs if paid other than monthly
  13. 2 months of pay stubs if paid monthly
  14. Medical bills
  15. Bring:  November, December and January income/paystubs if requesting back pay for prior medical bills

HealthWave 21 was created for uninsured children, ages 0 - 19, living in households with income levels at or below 200% of the federal poverty level. Within the first year, 15,500 of those children and teens were found eligible for HealthWave 21. In addition, through HealthWave Outreach efforts, an additional 17,000 previously uninsured children were found to be eligible for Kansas Medicaid also known as HealthWave 19. As of the end of 2005 over 37,000 children were eligible for HealthWave 21 benefits
